When You
First Call
We will discuss your concerns and recommend a counselor to best help the you meet
your needs:
If you are calling for a particular counselor, you may request that person and schedule an appointment with him or her.
The Intake Counselor will inform you that Samaritan Counseling Center can accept most major insurance coverage, however we do not accept any HMO's.
A sliding fee is available for those who can’t afford the full cost of counseling or who do not have insurance coverage.
Bring in your
insurance card. (if applicable)
When You First Meet with the Counselor
The first session is an intake session. It is important for the counselor to understand your concerns and history, and complete necessary paperwork:
In the first appointment, the counselor will ask you to:
complete the Intake Form that gives the Clinic general information about the you;
fill out and sign a Release of Information Form that permits the counselor to consult with the your doctor and/or pastor. (This is optional, and you complete the Release form only if you want the counselor to get medical records or consult with the your doctor or pastor);
provide insurance information (if applicable), and sign a fee agreement.
explain your rights;
develop with the you a treatment plan that identifies the your concerns and the outcome the you would like to receive from counseling;
give an estimate of the length of treatment to achieve the your desired outcome;
discuss with the you any applicable or necessary tests or assessments.

about treatment and/or your counselor’s training and treatment approach.
